(b) BY POST : From the Office of the Director (PMT-Cell), Institute of Medical Sciences, Banaras Hindu University, Varanasi-221 005 by sending a written request through Registered Post along with Crossed Demand Draft/Bankers Cheque (MICR only) of Rs. 1600/- for General/OBC candidates and Rs. 1100 for SC/ST candidates of a Nationalized Bank, drawn in favour of “The Director, Institute of Medical Sciences, BHU” payable at Varanasi. Application form will be sent by Speed / Registered Post.
However, Institute will not be responsible for any loss or postal delay. The request for application form must include the name and address of the candidate, and full particulars of the enclosed DD/BC (write candidate’s name and address on the back of DD). Two self-addressed, preferably typed, white paper slips of 10 cm 6 cm size must also accompany it.
The requisition for PMT Application Form should be superscribed “Request for Application Form for PMT-2012, BHU”. Last date of requisition for PMT form by post is 27.02.2012. Incomplete/non-cashable Draft (under amount/unsigned/ without Bank Code No. & Seal) shall not be accepted. NOTE: (a) M.O./I.P.O./Cheques shall NOT be accepted under any circumstances. Application Forms from the Office of the Director (PMT-Cell), Institute of Medical Sciences, BHU, Varanasi-221005 can be obtained by POST only and not in person upto 27.02.2012.
LAST DATE FOR SUBMISSION OF COMPLETED APPLICATION FORM : Application form duly completed must be sent through Registered Post so as to reach THE DIRECTOR (PMT CELL), INSTITUTE OF MEDICAL SCIENCES, BANARAS HINDU UNIVERSITY, VARANASI-221 005 latest by 15.03.2012. Application forms received after the last date shall not be accepted. Foreign Nationals are not required to appear in the PMT examination for MBBS/BDS/BAMS courses. They may seek admission through the Ministry of External Affairs, Govt. of India, New Delhi, as per the rules laid down for the purpose and their nomination must reach by 15.07.2012. Educational Qualification: Passed Intermediate / 10+2 or equivalent examination in English, Physics, Chemistry and Biology (Zoology + Botany) taken together
(i) For MBBS/BDS/BAMS – a minimum of 50% marks for General and 45% for General-PC candidates and 40% marks for OBC#/SC/ST and respective PC candidates;
(ii) For B.Pharm. – a minimum of 60% marks for General / OBC and respective PC candidates and 55% marks for SC/ST and respective PC candidates. Candidates appearing in qualifying examination may also apply.
AGE: Completed 17 years but not crossed 25 years as on 31st December, 2012 (Candidates born after 1st January, 1996 and on or before 31st December, 1987 need not apply).
Admission of Foreigners: Foreign nationals are not required to appear in the BHU PMT 2012. Such candidate may seek admission through the ministry of external affairs, govt of India, New Delhi, as per the rules laid down for the purpose and their nomination must reach by the July 15, 2012
DISTRIBUTION OF SEATS
Three percent seats are reserved for Physically Challenged (PC) candidates on horizontal basis in the above courses.

BHU PMT is a two stage examination; i.e; BHU PMT Screening and BHU PMT Mains. Candidates willing to study MBBS or BDS course at IMS BHU have to appear for both the tests. However, for admission into BAMS and B.Pharm courses merit list is prepared on the basis of BHU PMT Screening Test only.
